(up to $4,000/month) CLIENT'S AVAILABLE HOURS Monday through Friday | 9:00a - 5:00p SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY We are seeking dedicated Case Managers to support individuals and families, managing a caseload of up to 35 clients. The Case Manager will provide direct services through home visits, phone calls, office meetings, and written communication. This role includes conducting outreach to raise awareness of agency programs, visiting schools to meet with staff and students, and collecting necessary documents. The Case Manager will also make appropriate referrals, accompany clients to appointments when necessary, and represent them in Regional Truancy Court and Family Court. Coordination with other agencies to connect clients with additional services is essential. The position requires maintaining accurate and up-to-date case files, including data entry and detailed case notes.
