Job Description Works from a multi-line centralized communication center, offering triage and health information to patients by prioritizing incoming calls based on clinical acuity. Responsible for assessing the needs of the patient, develops a care plan based upon phone triage assessment, and directs patients to the appropriate healthcare provider/facility if required. - Schedule: - 8 hour shifts - Days / Evenings - As scheduled between 7:00 am - 11:00 pm - Every other Weekend & Every other Holiday - Note: This is not a Remote/Work-From-Home position. Sign-On Bonus!: In addition to competitive pay & our Total Rewards benefits, we are currently offering a $6,000.00 sign-on bonus to eligible RNs with a 2-year commitment. Responsibilities: - Provides patient education regarding disease management and medications, and anticipatory guidance and counseling. - Provides healthcare recommendations based on assessment of patient needs using approved protocols. - Processes prescription refills - Know and follow all organizational/departmental policies/procedures - Assesses patients by eliciting a focused health history. - Documents accurately all call inquiries, communicating complex triage calls with Director and/or physician and takes verbal orders. - Assists with statistical data collection - Assume responsibility for personal development and education. - Provides healthcare information to patients through telecommunications media using the nursing process. - Communicates with other departments and pharmacies regarding patient needs, as necessary. - Orders labs/testing and schedules appointments with the office as needed - Certificate in Nursing required. - 3 years of experience as a Staff RN required. - EPIC Experience preferred. - Note: This this is not a remote / work-from-home position. Licensure, Certifications, and Clearances: - Act 34 - Cardiopulmonary Resuscitation (CPR) - Registered Nurse (RN) - Registered Nurse licensure in the Commonwealth of Pennsylvania required - Current Pennsylvania nursing license, or a multistate license issued by an NLC?participating state. Employees working on an out?of?state NLC license who later establish residency in Pennsylvania must apply for a PA Nursing license within 60 days of their residency change.
