care.QualificationsRequired:
- BSN
- Graduate of an accredited school of nursing required
- A minimum of three (3) years experience, with at least two (2)
years in cardiologyPreferred
- Knowledge of standard of care treatment for heart failure
- Strong cardiac background
- Certification in Cardiac NursingCertifications And Licenses

- Active NJ RN license or compact RN license with NJ
endorsement
- BLS from the American Heart Association
- ACLS from the American Heart AssociationScheduling Requirements

- Monday FridayEssential Functions
- The Clinical Navigator performs patient assessment, facilitates
coordination of services and care, including: patient assessment,
patient, family, staff and community education programs, and
documentation.
- Assists in the development of clinical practice guidelines,
usage tracking, MCS database, new project/device implementation and
participates in program related quality assurance/performance
improvement activities.
- Participates in intra and interdisciplinary programs and
committees, attend organ selection committee meetings, quality
assurance performance improvement (QAPI) review meetings,
departmental education meetings, and participate in hospital
education to staff as warranted, which may include annual
competency training and/or initial orientation.
- Edit, and maintain the electronic medical records and
regulatory electronic data submission elements. Compile, calculate,
and analyze statistical, medical, and operational data to report to
government, regulatory, and medical organizations. Ensure the
accuracy and completeness of required regulatory care plans and
organ sharing forms (if indicated).
- Track quality data indicators and provide-on-time monthly
reporting for department QA and for Intermacs.
- Serves a a liaison between the patients, families and health
care team during hospitalization and the post discharge period
- Communicates to Cardiologist, Heart Failure Director, and
hospital staff regarding abnormal findings in the evaluation of
patients
- Works with the hospitalists, cardiologists, and HF Director in
assessing readiness for discharge and risk for readmission
- Responsible for the guidance and coordination of care for
patients and their caregivers through the continuum of care for
both inpatient and outpatient
- Provides individualized information, support and education
- Works with case management and heart failure team to coordinate
appointments for consultations and physician visits
- Participates in interdisciplinary nursing rounds for inpatients
- discharge planning process
- Responsible for performance improvement and other metrics to
determine effectiveness with all level of care providers to
optimize the patient care experience
- Revises and monitors educational materials to ensure that they
are coordinated, understandable, and meet the program
standards
- Maintains the materials based on evidence and best practice
guidelines
- Demonstrates a commitment to patient, visitor, and staff by
complying with all applicable safety regulations
- Learning the impact of medical errors and methodology that will
lead to reduction of errors
- Reporting actual and potential errors, as well as hazardous
conditions
- Identifying opportunities to standardize processes and 'error
proof' systems that will lead to increased safety
