Job Description:
Seeking a compassionate Registered Nurse Case Manager to serve
patients and families in Wichita and surrounding areas. The RN will
conduct assessments, develop care plans, and provide
comfort-focused nursing care, responding promptly to calls,
visiting patients, and managing after-hours admissions and deaths.
Preferred experience in hospice, home health, or geriatrics; $3,000
sign-on bonus.
Responsibilities include coordinating with physicians and staff,
maintaining communication and documentation, and updating patient
records in compliance with Medicare and policies.
Educate patients and caregivers on medication, personal care,
safety, and end-of-life preparations.
Identify and address patient/family issues, participate in team
meetings, and provide support at death.
Ensure 24/7 coverage, evaluate hospice aides biweekly, and assess
LPNs monthly.
Develop understanding of hospice philosophy and pain management
through ongoing learning. All other duties as assigned.
