Case Manager 109

Salary $44,000

Description

Provide case management services to families and individuals residing in Permanent Supportive Housing.
The position includes completion of intakes, developing service plans, linkages to other services including, conflict de-escalation, managing relationships with a broad range of community providers, promoting housing retention through teaching life skills, setting up programming for tenants.
Utilization of HMIS case management software. Bi-lingual encouraged to apply. Ability to work independently.

Requirements

Bachelor's degree (B.A) from an accredited college or university, preferably in Social Work or a similar Human Service field is preferred.
Evening hours 2 nights a week and flexibility in creating own work schedule and may include additional on-call hours.
Driver's license with acceptable driving history.
